Job summary:
The registered nurse will be responsible for teaching Diabetes Self Management Classes in both in-person and virtual modes and is responsible for keeping abreast of Standards of Care for Diabetes and updating educational materials as appropriate. The registered nurse provides nursing care in the health care system within the scope of practice defined by the Nurse Practice Act and regulatory standards. The registered nurse assures safety in practice by maintaining a realization of own individual level of knowledge and experience and assumes responsibility for advancing knowledge of self and others in the patient care team by communication with and utilizing resource persons and programs available. The registered nurse collaborates with the clinical team and other patient care providers. Responsible for consistently setting and evaluating priorities. The registered nurse will participate in unit performance improvement activities and monthly meetings. The registered nurse uses an understanding of the principles of growth and development to assess each patient's age-specific needs. Utilizes the nursing process to integrate all aspects of patient care using appropriate interventions with measurable outcomes toward improvement.

Qualifications:
Required:

• Must be a graduate from an accredited school of nursing
• Successful completion of all orientation programs

C ertifications and Licenses Required:

• Current Registered Nurse License in the state of New Jersey
• Certified Diabetes Care and Education Specialist CDCES preferred or interest in obtaining CDCES

Scheduling Requirements:

• Per Diem, On Call
• Day or Evening shift
• 10 hours a month
• Sept - June
• Hours: 9-12 and 5:00pm - 8:00pm

Essential Functions:

• Collects information from the Participant Self Assessment of Diabetes Management form and supportive healthcare members to assist in the evaluation process and determining patients' educational needs
• Communicates essential patient care information within appropriate timeframes to appropriate individuals and remains available for questions
• Completes mandatory requirements, competencies and education net learning offerings in a timely manner
• Completes documentation for designed unit in timely and efficient manner
• Complies with unit and department scheduling guidelines
• Identifies problems and issues related to patient care and department needs and follows protocol and policy to resolve ie incident reporting
• Integrates psychosocial, physical, nutritional and rehabilitation needs of the patient in patient care
• Maintains the standards of nursing care which are evidence-based and outcome-driven and implements the policies and procedures of the Ambulatory Care Center
• Participates in performance improvement initiatives and completes assignments,
• Performs evaluation, assessment and reassessment of patients according to applicable department standards utilizing designated tools and systems
• Performs interventions in accordance with applicable policy and procedure, safety guidelines and infection control principles
• Transmits pertinent patient information to the appropriate individual
• Utilizes the nursing process with a multidisciplinary approach in the delivery of patient care
