This Mental Health Worker role within the Involuntary Outpatient Commitment (IOC) program offers opportunities for professional growth, interdisciplinary collaboration, and a culture committed to recovery, dignity, and community stability. • Developing and updating individualized, person-centered rehabilitation plans in collaboration with consumers, families, and the interdisciplinary care team
  • Coordinating and facilitating access to behavioral health treatment, medical care, housing, transportation, benefits, and other community resources
  • Delivering care coordination, health promotion, supportive counseling, crisis intervention, and transitional care services to promote stability in the community
  • Assisting consumers with treatment enrollment, establishing community supports, and connecting them with social service agencies and recovery resources
  • Collaborating closely with psychiatrists, nurses, social workers, family members, and community providers to ensure coordinated, consumer-centered care
  • Maintaining accurate, timely documentation that meets billing, regulatory, and organizational standards
  • Providing transportation for consumers as needed to support access to treatment and essential services
  • Supporting the department's mission of helping consumers achieve recovery, maintain community stability, and progress toward greater independent living

This role might be for you if:

  • You enjoy collaborating with interdisciplinary teams to develop individualized care plans
  • You value culturally appropriate, person-centered care and believe in empowering consumers to reach their personal goals

Qualifications:

To be considered for this Mental Health Worker opportunity, you are committed to providing high-quality community-based behavioral health services while helping consumers remain safe, engaged in treatment, and supported in their recovery.
