Key Responsibilities

• Conduct initial and periodic physical therapy evaluations to determine residents’ functional status and establish appropriate treatment plans.
• Implement individualized treatment plans based on physician orders, assessment findings, and resident-centered goals. Deliver skilled physical therapy interventions to improve residents’ mobility, strength, balance, endurance, and functional independence.
• Modify and progress treatment activities based on resident response, with a focus on maximizing functional outcomes and promoting safety and quality of life.
• Accurately document treatment sessions, resident response, and clinical observations through daily notes, progress reports, and discharge summaries in accordance with facility policy and regulatory standards.

Requirements

Qualifications

• Education: Graduate of an AMA-approved Physical Therapy program
• Licensure: Active New Jersey State Physical Therapy License
• Experience: Minimum 3 years in a rehabilitation setting
• Technical Skills: Physical therapy evaluations, treatment planning, documentation
• Soft Skills: Patient-centered approach, strong communication, adaptability
