Shift: Full-Time, Monday – Friday, 8:30 AM – 3:30 PM

About Us and Our Program:

Easterseals, a national nonprofit, has supported individuals with disabilities for over 50 years. Its community mental health clinic, Centerpath Wellness, provides mental health, addiction, and wellness services to children and adults throughout New Jersey.

What You’ll Do:

The Partial Care Counselor works as an integral part of a multi-disciplinary team approach to the development and maintenance of a therapeutic community toward the rehabilitation of target populations.

Key responsibilities include:

· Provides individual, group, family, and psychoeducation to clients and their families.

· Reviews, updates, and utilizes the biopsychosocial assessment to develop and implement treatment plans.

· Prepares a comprehensive treatment plan and updates as required.

· Provides basic case management services and discharge planning.

· Uses discretionary judgement in escalating a client’s safety and well-being.

· Provides and arranges transportation for clients as needed.

· Conducts outreach calls or written letters to clients to maintain client care and adhere to regulatory compliance.

Who You Are:

· Education: Associate’s degree preferred.

· Experience: Minimum of 2 years of experience in human services.

· Requirements: Must possess valid driver's license and acceptable driving record.

· Skills: Computer proficiency with MS Office and Electronic Health Records.
