We’re seeking a Bilingual Intake Coordinator (English/Spanish)to help patients transition from care facilities to our personalized pharmacy service — making their healthcare journey easier, safer, and more personal.

Your Role as a Bilingual Intake Coordinator

As an Intake Coordinator , you’ll be the friendly, empathetic voice that helps patients transition smoothly from care facilities to the comfort of their own homes — ensuring they receive their medications correctly and on time.

Key Responsibilities:

• Contact patients preparing to leave or who have recently left care facilities
• Introduce PersonalRX and guide patients through our pharmacy service
• Review patient profiles from facilities for accuracy and completeness
• Arrange and confirm delivery of upcoming prescriptions (“second fills”)
• Keep patients informed and answer questions about medications or deliveries
• Coordinate with pharmacy, billing, and insurance teams as needed
• Manage call queues and hand off cases to Personal Care Coordinators
• Document all interactions accurately and follow up when necessary

What We’re Looking For

• Bilingual in English & Spanish (fluent reading, writing, and speaking)
• 1–2 years of customer service or patient-facing experience
• High school diploma (required)
• Excellent phone communication and interpersonal skills
• Strong organization and attention to detail
• Comfortable using computer systems and handling multiple tasks at once
• Experience in healthcare or pharmacy a plus
• Compassionate, patient, and professional demeanor
