Overview
The Pharmacist performs pharmaceutical care activities for customers/patients within the service area; monitors, documents and reports patient outcomes; provides or coordinates the dispensing of medications, home medical equipment and ancillary supplies within the service area; and participates in on-call schedule.
Responsibilities
- Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals
- Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice
- Adheres to all appropriate standards of accreditation and participates in quality assurance activities
- Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution
- Reviews, receives and verifies all new patient orders and documents on the respective order form
- Assists the physician and other involved healthcare professionals with: Planning and implementing patient care in the home
- Setting therapeutic objectives for drug therapy
- Establishing parameters for monitoring the safety and efficacy of drug therapy
- Preparing regular progress reports
- Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
- Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
- Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
- Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
- Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
- Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences.
- Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules

Qualifications
EDUCATION/EXPERIENCE
• Bachelor of Science in Pharmacy
• Doctor of Pharmacy preferred
• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy
• Experience with compounding infused drugs
LICENSE/CERTIFICATION/OTHER SPECIAL REQUIREMENTS
• Registered Pharmacist License in state(s) of practice
• Willing to obtain and maintain compliance with additional state licensure outside of home state, to support company operation
KNOWLEDGE/SKILLS/ABILITIES
• Good organizational and time-management skills
• Participation and membership in professional societies and organizations is desirable
• Excellent verbal and written communication skills
• Schedule flexibility
TRAVEL REQUIREMENTS
Percentage of Travel: 0-25%
**To perform this role will require frequently sitting, standing, walking and typing on a keyboard with fingers, and occasionally bending, reaching, and climbing (stairs/ladders). The physical requirements will be the ability to push/pull and lift/carry 1-10 lbs**
