Review discharge instructions and ensure understanding
Determine if there are questions or concerns following the hospital stay.
Escalate issues or concerns by notifying appropriate parties (e.g. Case Management,
Physician, Clinical Director, Patient Relations, etc.)
Summarizing responses in a database for documentation, trend analysis and follow-up. Experience Required 0-2 Years Bilingual Preferred (Spanish) Education Requirements High School Diploma Required Bachelor's degree Preferred Create a Job Match for Similar Jobs
