PACT Wellness/Housing Specialist 067 Salary: $42,000 per year plus Sign On Bonus - $3,000 Responsibilities: Works as part of a multi-disciplinary PACT team Provides community-based treatment and skills training to persons with serious mental illness Responsible for identifying, locating, and securing housing for individuals with mental illness Works closely with state psychiatric hospitals to facilitate discharges of individuals into the community.
