Pride Health is currently seeking a compassionate and experienced Social Worker (MSW) for a hospital-based contract role in Morristown, NJ . In this role, you will provide therapeutic interventions, discharge planning, and connect patients and families with critical resources to ensure continuity of care. You will collaborate with an interdisciplinary healthcare team to support safe transitions from hospital to community settings.
Title: Social Worker
Location: Morristown, NJ
Shift Time: 8:00 AM – 4:00 PM | Weekend requirement
Contract Duration: 13 weeks (High possibility of extension)
Required Skills & Experience:
- Current NJ Social Work License (Active)
- Master’s Degree in Social Work (MSW) – Required
- Minimum 2 years of in-patient, hospital-based social work experience
- Proven expertise in psychosocial assessments and discharge planning
- Excellent knowledge of community resources and social determinants of health
- Exceptional communication, advocacy, and problem-solving skills.
