Physician Assistant - Vascular Surgery
Our Vascular & Cardiac Integration Program delivers advanced bedside evaluation and coordinated
specialty-driven care directly to skilled nursing facilities. This model reduces unnecessary hospital
transfers, accelerates diagnostic turnaround, and improves outcomes through a streamlined, on-site
vascular and cardiac service line.
Vascular & Cardiac IntegrationPosition Overview
We are seeking a New Jersey–licensed NP/APN or PA to serve as the primary clinical provider for
vascular and cardiac consults across partnered long-term care facilities.
The provider will conduct bedside assessments, order appropriate diagnostics, perform pacemaker
checks, collaborate with our specialty intervention team (vascular surgery, cardiology, or interventional
specialists), and provide structured follow-up care.
No procedures are performed by the NP/PA
No weekend or Sunday responsibilitiesKey Responsibilities
Clinical Evaluation & Rounding
Perform bedside assessments for vascular and cardiac consults within 24–48 hours.
Evaluate circulation, wounds, edema, cardiac symptoms, abnormal pulses, and related concerns.
Document all patient encounters in facility EMRs (PointClickCare/SigmaCare).
Diagnostics & Care Planning
Order ABI/PVR studies, vascular duplex, carotid/venous studies, EKGs, and echocardiograms.
Review diagnostic results and coordinate care plans with the supervising vascular/cardiac
intervention team.
Pacemaker & Device Interrogations
Perform routine pacemaker and cardiac device checks (training provided if needed).
Review battery status, impedance, rhythm logs, and alerts.
Communicate findings to the supervising cardiac or vascular specialist team.
Care Coordination
Collaborate with the coordination and transport team to schedule patients needing vascular or
cardiac interventions performed by the specialty physician team.
Maintain seamless communication with SNF nursing staff, attending physicians, and families.
Follow-Up & Ongoing Management
Conduct structured follow-up rounds after specialty interventions.
Monitor wound progress, vascular circulation, cardiac optimization, and medication adherence.
Support continuity of care for high-risk patient
Qualifications
Valid New Jersey NP/APN or PA license.
Background in cardiology, vascular medicine, wound care, internal medicine, or geriatrics
preferred.
Ability to perform or be trained in pacemaker/device interrogation.
Strong bedside assessment and independent decision-making skills.
Excellent communication and documentation abilities.
Schedule
Monday–Friday, flexible rounding based on consult volume.
No nights, no weekends, no Sundays.
