*Advance Practice Provider needed for a Skilled Nursing Facility, Wound Care
*Deliver high-quality, patient-centered care including assessments, exams, and treatment planning
*Perform procedures such as injections, immunizations, suturing, and wound care. Must be wound and ostomy certified
*Educate and counsel patients on treatment plans, wellness, and preventive care
*Collaborate with physicians and care teams to support continuity of care
*Salary: $95,000 – $150,000 + benefits
