Family Medicine Physician

Community Health Center (CHC) offers comprehensive, community-based, preventive, primary care, and wellness services for New Jersey residents of all ages at four convenient locations. CHC health care services are designed to meet the specific needs of the entire family from annual check-ups and immunizations to sick visits, mental health counseling, and critical prenatal care.

We are seeking a mission-driven Physician to join our team at the VNACJ Community Health Center in Keyport . In this role, you will provide holistic primary care to individuals and families of all ages, ensuring access to essential health services through collaborative teamwork. You will work alongside a dedicated team to drive clinical excellence, contribute to quality improvement initiatives, and support community outreach efforts. Responsibilities

• Support the organizations mission by striving for excellence in all aspects of your job with a focus on positive interpersonal relationships with co-workers.
• Provide direct family and patient-centered medical services and consultations to primary care patients and families at the sites within established productivity guidelines.
• Maintain complete patient records using the Electronic Medical Record system and comply with all reporting mechanisms.
• Ensure compliance with the New Jersey Department of Health and Senior Services Ambulatory Care Regulations for licensure, as well as requirements of other licensing and accrediting bodies, health maintenance organizations, and other insurance carriers.
• Collaborate in team-based care within the Patient-Centered Medical Home model.
• Participate in peer review and other quality improvement activities.
• Contribute to in-service education for staff.
• Adhere to CHC infection control and safety policies, and follow guidelines of regulatory agencies.
• Participate in community outreach programs regarding CHC services and programs.
• Supervise staff in conjunction with the organization's managerial and leadership practices.
• Perform other duties that are similar, related, or logical assignments for this position if omitted from this list.

Qualifications

• Licensed as a physician in New Jersey with education and experience in primary care settings.
• FQHC experience is preferred.
• Board-certified in family practice.
• Active CDS license.
• Bilingual (Spanish/English) skills are preferred.
• Valid NJ drivers license and Car.

Working Conditions/Physical Demands : Working indoors; exposure to blood, body fluids, and communicable diseases. Physical activities include sitting, fine motor hand movements, standing, walking, lifting up to 40 pounds, pushing, reaching, and pulling.
