Job Summary:
The Primary Care Nurse Practitioner (NP) will provide comprehensive primary care services to residents within a skilled nursing facility. The NP will function as an integral member of the interdisciplinary team, delivering high-quality, patient-centered care, focusing on preventative medicine, chronic disease management, and acute care interventions. The NP will work autonomously within their scope of practice, collaborating with physicians, nurses, therapists, and other healthcare professionals to optimize resident health and well-being.
Responsibilities:
- Direct Patient Care: Conduct comprehensive physical assessments, obtain medical histories, and develop individualized care plans.
- Diagnose and treat acute and chronic illnesses, including but not limited to cardiovascular disease, diabetes, respiratory conditions, and dementia.
- Order and interpret diagnostic tests, such as laboratory studies, radiographs, and electrocardiograms.
- Prescribe and manage medications, adhering to established protocols and guidelines.
- Perform minor procedures, such as wound care, suture removal, and injections.
- Provide palliative and end-of-life care, ensuring resident comfort and dignity.
- Chronic Disease Management: Develop and implement strategies for managing chronic conditions, including regular monitoring and adjustments to treatment plans.
- Educate residents and their families on disease management, medication adherence, and lifestyle modifications.
- Monitor and evaluate the effectiveness of interventions, making necessary adjustments to optimize outcomes.
- Preventative Care: Conduct routine health screenings and physical examinations.
- Administer immunizations and provide preventative health education.
- Identify and address risk factors for common geriatric conditions.
- Collaboration and Communication: Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Communicate effectively with residents, families, and healthcare professionals, ensuring clear and timely information exchange.
- Document all patient encounters and interventions accurately and thoroughly in the electronic health record (EHR).
- Participate in care plan meetings.
- Communicate with pharmacies, and other outside providers.
- Quality Improvement: Participate in quality improvement initiatives to enhance resident care and safety.
- Stay abreast of current evidence-based practices and guidelines.
- Adhere to all regulatory requirements and facility policies.
- Supervision: May provide oversight to other nursing staff as required.
Qualifications:
- Current and unrestricted Nurse Practitioner (NP) license in the state of NJ
- Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or other relevant specialty.
- Master's or Doctoral degree in Nursing.
- Minimum of 2 years of experience as a Nurse Practitioner, preferably in a geriatric or long-term care setting.
- Strong clinical skills and knowledge of geriatric medicine.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and collaboratively within 1 an interdisciplinary team
- Proficiency in electronic health records (EHR) systems.
- Current CPR certification.
- Ability to pass a background check.
Preferred Qualifications:
- Experience working in a skilled nursing facility.
- Certification in geriatrics.
- Experience with wound care.
Physical Requirements:
- Ability to stand, walk, and sit for extended periods.
- Ability to lift and transfer residents as needed.
- Ability to bend, stoop, and reach.
- Visual and auditory acuity sufficient to perform job duties.
