Job Summary:
The Nurse Coordinator RN in Primary Care practice provides guidance for patients and families seeking services by serving as an active liaison between patients and families, medical subspecialists, therapies, insurance companies and community medical agencies within a family centered care model. Assist families in finding the proper resources to supplement medical care and quality of life. Interact with social services and community agencies. Support family through provision of resources and support networks as appropriate.
Qualifications:
- Formal Education and Job Related Experience
- Bachelors degree in nursing preferred
- Case management experience preferred
- Minimum of one years experience in pediatric setting
- Excellent communication skills, both verbal and written
- Excellent Interpersonal skills
- Good planning and organizational skills
- American Heart Association BLS Provider Certification required
- Current New Jersey RN license
- Must be comfortable working with children with a wide variety of complex medical needs
- Excellent cognitive analytical and problem solving skills are essential
- Must be self motivated and self directed
- Must be able to manage multiple tasks simultaneously
Essential Job Functions:
- Coordinates care for children receiving outpatient services as well as respite and inpatient rehab,
- Coordinates communication between patient and family, internal and external service providers,
- Demonstrates an understanding of family centered care, patients medical diagnoses, prognosis, progress and implications for family support and education,
- Assists families with navigation related to medical services needs and reviews materials with multidisciplinary team as needed,
- Engages in advocacy for patient and empowered caregivers to assume independent skills in seeking and obtaining support and services for child,
- Demonstrates an understanding of the health care system, managed care and resource utilization,
- Refer patients and families to home care agencies related to private duty nursing, personal care assistance, and in home therapies,
- Advocates for the unmet needs of patients families across medical continuum,
- Provide necessary documentation to external agencies as needed including letters of medical necessity, WIC forms, care plans, FMLA documents, DME orders, and out of network authorizations,
- Effectively communicates financial and social needs, barriers, and issues to the multidisciplinary team, physicians and external agencies,
- Identifies families at high risk for child neglect or abuse and makes appropriate referrals,
- Be a resource for families to ease them through transitions and hardships,
- Acts to enhance understanding of CSH services and mission through development of relationships with community, service providers, and referral sources,
